Who Is Eligible to Receive Medicare?

Medicare is available for seniors ages 65 and older and permanent U.S. residents with qualifying disabilities and chronic medical conditions

Benefits of value-based care

You may see your primary care doctor more often - they are the quarterback for all of your care and will help you proactively manage chronic conditions.

Your care will be coordinated across all places where you see doctors. For example, you will not have any unnecessary tests or blood work, as results will be shared with all of your doctors.

Your doctor partner with Medicare Advantage plans and have access to data about the care you receive outside their office.

Patient Testimonials

“We don’t have any doctor patient situation, it’s a family. It’s a family situation where we come in and he takes over checking our files, giving us good advice, but from the heart not from a doctor. The doctor, he is in the room with us but he is not just the doctor, he’s a caregiver and a good, good friend.”

Richard and Betty, Pompano Beach Clinic

“Honestly, sometimes it feels like a brother or somebody I have known all my life. I can call him today and say I’ve got something wrong, and I guarantee you, I will be in the office before the end of the day. I have never had it any other way”

Ken, Hudson Clinic
